The film begins in England during the Dark Ages, where an endless war between man and creatures of darkness left King Arthur with no choice. With his powerful wizard, the King goes to Pendle Hill, and beneath an ancient oak tree, he offers a truce with the immortal witch and the Blood Queen. However, this is only a trap for the Blood Queen, as King Arthur believes that mankind and her kind are never meant to live together in harmony.

King Arthur ambushes the Blood Queen with the help of her trusted witch and Excalibur. Using the sword, King Arthur dismembers and beheads the Blood Queen before placing her body parts in separate caskets, sealed with a holy prayer that can only be broken with the words of a man of God. His knights then scatter these caskets to the far reaches of the land.

The scene then transitions to the present day, where Hellboy arrives in Tijuana to find an agent sent by the Bureau for Paranormal Research and Defense to investigate a nest of vampires. With the help of a reliable source, Hellboy finds the agent in an arena, where he fights as a wrestler. The agent challenges Hellboy to a fight, and only then will he agree to return to the paranormal bureau.

As they fight, Hellboy repeatedly attempts to snap the agent out of his character, but upon seeing the long nails and fangs, he suspects the worst. He headlocks the agent and removes the mask, revealing that the agent has been turned into a monstrous vampire. The patrons run after seeing the agent's true form, while Hellboy fights and accidentally impales him on a post. Hellboy rushes to the agent after realizing his mistake, and the agent returns to normal.

With his last strength, the agent calls Hellboy by his true name before mumbling in Spanish that the end is coming. The Professor, Hellboy's foster father, sends two agents to fetch Hellboy, who is seeking solace in alcohol after having his friend die in his arms. As Hellboy returns, the professor tells him that the Osiris Club in England wants his help to hunt down giants.

Just like their organization, the Osiris Club fights against the forces of darkness, and this British occult group is a close friend of the professor. On the other side, a warthog-like creature visits an old woman monster with a severed right eye named Baba Yaga. Since both have suffered great pain at the hands of Hellboy, Baba Yaga orders the warthog to assemble and free the Blood Queen to gain the revenge they both seek.

Hellboy travels to England to the Osiris Club's headquarters, where he is welcomed by Lord Adam and his associates. Lord Adam shows Hellboy a room full of giant heads they have collected every time they hunt. It turns out that three giants have risen from their graves and are now wreaking havoc; the club wants to stop this trio before they reach the city. There are also photographs of the club members hunting centuries ago, which catches Hellboy's attention since the professor and Lord Adam still look young for their age.

Lady Psychic, a woman gifted with foresight and the club leader, then appears. She reveals to Hellboy that four club members—including her, Lord Adam, and the professor—were chosen by a powerful spirit to destroy something that would end mankind during World War II. Ever since, they have aged at a snail's pace to fulfill that mission. The object of their concern turns out to be Hellboy. Lady Psychic explains Hellboy’s origins, prompting a flashback.

During WWII, Rasputin and his minions attempt to bring something from another dimension to use as a weapon, as they are losing the war. However, the ritual doesn't turn out as planned. Fortunately, legendary Nazi hunter Lobster arrives with the paranormal bureau, kills the Nazis, and executes Rasputin. The club members are instructed to kill whatever abomination the Nazis summoned in the depths of hell. However, when the professor sees the cute baby Hellboy, he chooses to adopt him.

As the flashback ends, Lady Psychic tells Hellboy that Rasputin brought him into the world as a weapon, but the professor used him for good. Meanwhile, the warthog starts his mission by infiltrating a church full of monks practicing their vow of silence. The warthog tears their flesh like pieces of paper but keeps one monk alive and forces him to guide them beneath the church. Fearing the creature, the monk leads him to a casket storing one of the Blood Queen's body parts.

The monk refuses to chant the incantation, so the warthog rips out his tongue and swallows it, chanting the incantation himself. Only the words of a man of God can break the holy seal, and the warthog succeeds, awakening the Blood Queen's head. The following day, Hellboy travels with the Osiris hunters to hunt the giants, only to be double-crossed. One by one, the hunters plunge their spears into Hellboy, who manages to kill some of them while running away.

However, when three spears are plunged into him, Lord Adam turns on an electric frequency, electrocuting him repeatedly while revealing that they never needed his help—it was just a trap to destroy him. Lord Adam takes out his sword and prepares to behead Hellboy, but a giant suddenly appears and beheads him instead. Meanwhile, the warthog successfully retrieves the Blood Queen's remains one by one, with only her left arm remaining to make her whole again.

Hellboy wakes up to find two giants feasting on the hunters' corpses. As he looks for the third giant, it appears behind him and tries to kill him with its gigantic sword. This catches the two giants' attention, and they start attacking Hellboy. Already wounded and weakened, the giants toss him around, but he manages to inflict some damage. One by one, Hellboy begins slaughtering the giants. He plucks the eyes of two and sticks a giant tree into one giant's throat.

He then moves on to the last one, cutting its limbs with its gigantic sword before slashing its head. As his vision blurs, Hellboy spots a van approaching before he passes out. He wakes up in an apartment, where he reunites with an old friend, Alice, who has the power to hear the voices of the dead. As they sit across from each other, Alice shares that the dead are whispering to her to kill him.

She adds that she has a shotgun under the table, loaded with angel bones, aimed right at Hellboy, waiting for her to pull the trigger and end it all. Hellboy sighs in relief when Alice lets go of the trigger and tells him to hide under the table. When asked why they are hiding, Alice casually tells him it’s to avoid the glass. Just in time, the professor's men break into the apartment to ensure Hellboy is okay.

As they walk in the hallway, Hellboy tells the professor about his so-called friends trying to kill him. However, the professor is less concerned with that and more with the fact that the Blood Queen is awake, and the last piece of her body lies in Osiris Club headquarters. The professor then introduces Hellboy to Major Daimio, who is working with the paranormal bureau to stop the Blood Queen.

The two immediately form a heated atmosphere, as Daimio doesn't want to work with a monster like Hellboy, who insults Daimio's facial scars. However, the professor orders them to work together with Alice to stop the Blood Queen from bringing her deadly plague to the world, one that could wipe out mankind in a short time. Before they depart, the professor hands a briefcase to Hellboy containing his new “hormone toy.”

On the way to the headquarters, Hellboy opens the suitcase, revealing a beautifully customized pistol. Shortly after, they reach the headquarters, where Alice experiences an excruciating psychic migraine from the screams of the dead. As they enter the mansion, Alice experiences another migraine after seeing that every club member has been brutally killed. They find Lady Psychic's body, whose head Alice touches so that her spirit can communicate with them.

Lady Psychic's spirit appears in Alice's mouth and warns Hellboy that now that the Blood Queen is awakened and searching for a King, his true destiny will be revealed and fulfilled. As Lady Psychic's spirit disappears, they hear growling in the distance, which Hellboy follows. He finds the warthog taking the Blood Queen's last piece with him, so Hellboy fights the warthog to stop him. However, the warthog calls out to his Queen, and a bright light reveals the Blood Queen in a vision.

She catches Hellboy's attention, and with her calming voice, she plays with his mind, manipulating his feelings about how creatures of darkness like her and him are treated in the human world. Simultaneously, the Blood Queen disappears when Daimio snaps Hellboy out of the vision, causing him to shoot at Daimio. Fortunately, the bullets miss, but Hellboy triggers something else. Daimio injects something to calm himself down before following Hellboy, who reveals his connection with the warthog.

The year is 1992, and Alice is just a baby. Babies like her are often stolen by fairies. Alice's parents seek Hellboy's help, as the mother is certain it is not her child anymore, replaced by a changeling. Hellboy coaxes the creature out of its disguise using iron, prompting a then-small warthog to reveal its true form. Hellboy brands the warthog and warns him to return Alice, or he will hunt him down. Later that day, the warthog returns Alice safe and sound from the fairies.

Since then, the warthog has wanted revenge, intending only to live and kill Hellboy. On the other side, the Blood Queen's witches stitch her body together while chanting, making her whole once again and ready to wipe out mankind. The trio goes to the headquarters the following day, using a fish-and-chips shop as a disguise. Daimio leaves Hellboy and Alice with his subordinates while he goes to acquire a special bullet designed to kill Hellboy. The bullet is cast from Judas's silver mixed with Saint Dominic's blood and blessed by the Holy Father himself.

At the headquarters, Hellboy snaps at his foster father, the professor, screaming how creatures like him deserve to live in peace if it weren't for humans. Hellboy then questions why the professor didn't kill him. The professor responds that he saw potential in him. However, Hellboy replies that he was made into a weapon to kill his kind before storming off. Hellboy gets into the elevator, only to be transported into another realm, where he finds Baba Yaga's house.

The witch brings Hellboy into her home, where she offers him a feast made of human children's remains. She offers to help him find the Blood Queen in exchange for one of his devilish eyes, which he had previously gouged out of her own eyes. Hellboy reluctantly agrees, and Baba Yaga seals the deal with a very gross and sticky kiss on the lips—but without using her tongue. After that, Baba Yaga tells him to go to Pendle Hill, where the Blood Queen will reclaim her blood to restore her power. Hellboy only has until midnight.

Baba Yaga then prepares to take her end of the bargain, when Hellboy tricks her, since they did not specify when he had to give her the eye. Baba Yaga curses him as he falls out of her house and back into the same office with the professor and Alice. Hellboy informs them of the Blood Queen's whereabouts before walking away. As the team heads to Pendle Hill, Alice persistently asks Daimio about his facial scars. Daimio reluctantly shares how he got them, prompting a flashback.

Daimio and his team are in Belize, where local tribal elders ask for their help to hunt a demonic jaguar. However, the creature is so fast that it kills Daimio's whole team within minutes, while he survives after being clawed in the face. Since then, Daimio has hated creatures of darkness. The team lands near the hill, where they fight creatures of darkness. At this time, the Blood Queen reclaims her blood from the ancient oak tree, restoring her full powers.

Her disloyal witch then shows up with her sisters, seeking forgiveness for betraying the Blood Queen. However, the Blood Queen kills her sisters with just her hand and keeps the betrayer alive, who presents her with a thorny crown. Suddenly, a bullet strikes the Blood Queen's left face, made by Hellboy's new toy. Simultaneously, Daimio and Alice fight the creatures of darkness, with the help of Alice's newfound power, punching the souls out of the dead.

The Blood Queen tempts Hellboy to join her so they can rule together, but Hellboy firmly refuses. Before escaping through a portal with the warthog, the Blood Queen takes a piece of thorn from her crown and throws it at Alice’s neck, poisoning her. The betrayer witch then meddles and instructs Hellboy and Daimio to take Alice to the wizard to undo the poison. With her knowledge, the team successfully reaches the wizard’s tomb and awakens him after centuries. With one look at the thorn, the wizard recognizes that it’s the Blood Queen’s work.

He tasks Hellboy with doing whatever it takes to kill the Blood Queen before removing the thorn. After saving Alice, the wizard knocks her and Daimio unconscious to have a private talk with Hellboy. The wizard slashes Hellboy’s hand and drops his blood onto something as he reveals the truth about Hellboy’s identity. It turns out King Arthur’s bloodline continued until it ended with a girl who married a demon in 1574. That night of their marriage, the girl was taken down to Hell, where she gave birth to a son, Hellboy.

This explains that Hellboy is half-human and the last royal of King Arthur’s bloodline. With Hellboy’s blood, the wizard summons Excalibur, the one weapon that can terminate the Blood Queen. Only a descendant of King Arthur can wield the sword. As soon as Hellboy grabs it, the sword shows a vision of him with his horns fully grown, riding a dragon, and coming down to Earth to wipe out mankind. Hellboy removes his hand from the sword, refusing to take it, fearing he will bring about the apocalypse.

The wizard disappointingly tells Hellboy he made a big mistake by refusing Excalibur, then crumbles to death, having used the last of his magic to summon the sword. Meanwhile, with her power restored, the Blood Queen unleashes her deadly plague upon London. It spreads rapidly, instantly killing many people as a start to wiping out mankind. At the same time, the Blood Queen invades the headquarters. Shortly after, Hellboy, Daimio, and Alice arrive, only to find everyone massacred and the professor missing. After seeing the Blood Queen and her creatures there, they head to St.

Paul’s Cathedral, where a now gigantic warthog welcomes the team. Hellboy and the warthog charge at each other and start fighting. Seeing the warthog dominating Hellboy, Daimio transforms into his were-jaguar form to help, but the warthog quickly knocks him out and causes massive debris to fall on Hellboy. The Blood Queen appears and stops the warthog, betraying him and shrinking him until he pops into a bloody mess. She then continues to tempt Hellboy, bringing him beneath the cathedral floor, where he finds King Arthur’s tomb and Excalibur resting in its stone.

Hellboy still refuses to wield the sword. The Blood Queen brings the professor and slices his neck open with a flick of her finger. Hellboy catches his father, begging him not to go, but it’s too late—he is already dead. Crowded by grief and rage, Hellboy grabs Excalibur, transforming into his full form, complete with horns and a flaming crown, opening a portal to Hell. Outside, colossal demonic creatures emerge, ripping people to shreds and laying waste to London, slowly fulfilling Hellboy’s feared destiny.

The Blood Queen, in awe of Hellboy’s Demon King form, offers herself as his Queen. Alice sneaks to the professor’s body to channel his spirit, snapping Hellboy out of the Blood Queen’s temptation. The professor’s spirit emerges, acting as the guiding and loving father figure Hellboy needs. As the Blood Queen tries to sway Hellboy, Daimio loads his gun with the special bullet, ready to take Hellboy down if he refuses to listen. The professor tells Hellboy he has always believed in him and loves his son.

With that, Hellboy decapitates the Blood Queen and plunges the sword into the ground, sending all demonic creatures back to Hell. Hellboy breaks his horns before taking the Blood Queen’s head and throwing it into Hell as the gates close, ensuring she can never return. Before disappearing, the professor tells Hellboy that being his father was the best decision he ever made.

Daimio destroys the bullet, telling Alice it was a mistake but not revealing what it contained. Six months later, Hellboy, Daimio, and Alice are in Siberia to face off against the Atlantis Society. Hellboy triggers Daimio’s were-jaguar form and attacks the villains with him. After defeating all members, the trio comes across a tank containing Abe Sapien, hinting at the next adventure.
